NHS CPFT Primary Care Mental Health Service

Back to Support

The Primary Care Mental Health Service (PCMHS) provides specialist mental health assessment within GP surgeries so people experiencing mental health challenges can access prompt advice and support, receive help in a community setting and experience a joined-up approach to care.

The Primary Care Mental Health Service used to be called PRISM. This video gives you an overview of the service:

This service is accessed via referral from your GP. The Primary Care Mental Health Service supports anyone aged 17-64 experiencing mental health challenges based on a person’s needs rather than their diagnosis.
